What is the coverage on exams and materials for the VSP Plan?

Exam - Once every 12 months
Lenses - Once every 12 Months
Frames - Once every 24 months

What are the co-pays for VSP Doctors?

Exam $15.00. Materials $25.00 (frames and lenses) when chosen from the VSP selection of frames. Optional Items are not covered and will be paid by the employee. Frames outside the selected frames are given an allowance towards the cost of the frame.

Can I go to any Optometrist?

YES, but by using a VSP doctor you will pay less out of pocket. "Out of Network" service require the covered individual to pay the entire bill, then file claim with VSP within 6 months from date of service.

Can I receive Glasses and Contacts in the same year? No. The plan pays for one or the other, not both.
