Step 9: Complete Foreign Employee
Information Form (Foreign Employees only)

All nonresident alien employees must complete the Foreign Nation Information Form (FNIF). The information provided will be used to determine eligibility for non-resident tax treaty benefits and income tax obligation while working for the University of Oklahoma.

There are separate forms for employees assigned to OU and OUHSC programs.

1. Complete the FNIF online.

2. Get copies of the following documents to attach to the FNIF:

  • I-94 Arrival/Departure record (both sides)
  • Passport
  • US Visa
  • I-20 OR DS-2019 OR I-797, if appropriate
  • Social Security card
3. Print, sign and date the FNIF. Attach copies of required documents.
