Initial Notice of Injury/Illness
Employees must notify their immediate supervisors or other appropriate supervisory personnel of their injuries as soon as possible.
Medical Treatment
University Employee Health Services, Family Medicine Green Clinic, 900 N.E. 10th Street, 271-3100, is designated first provider for medical treatment of job-related injuries and illnesses.
The supervisor will complete an Initial Treatment Acknowledgment and provide it to the employee when treatment is sought at Green Clinic.
After-hours or off-location medical treatment
The employee may seek treatment at any appropriate treatment facility, including an urgent care facility or, if necessary, an emergency room, after-hours or for off-location injuries.
Reporting
Access OUHSC workers compensation forms here. Instructions and responsibility for forms completion is as listed below. Forms should be completed the day of the injury or the next workday:
- Employee’s Report of Injury is submitted to the supervisor or department upon completion by the employee
- Supervisor’s Report of On-the-Job Injury/Illness is submitted to the department upon completion by the supervisor
The department representative will email all forms and any fitness for duty reports to HR Workers Compensation.
Return to Work
Return to work after absence due to injury is allowed only upon the University’s receipt of a written attending doctor’s release.
