Vision Insurance

Employees may participate in a vision care plan from VSP by paying the full monthly premium through payroll deduction. There is a standard vision plan and a premium vision plan. Both vision care plans provide coverage for prescription lenses and frames, contact lenses (in lieu of eyeglasses), and a complete annual eye exam. Under the plans, you can visit an optometrist in the VSP network or you may choose to visit an out-of-network provider, which may result in higher out-of-pocket costs.

VSP Member ID Card

VSP Member Card

A VSP ID Card is not required to obtain vision services. To use your VSP benefits, simply give your provider your health ID number, which is 990+ your employee ID, or your social security number. 

Vision ID cards are available online for you to download and print at your convenience. Cards contain your name, VSP's claims submission address, website and customer service telephone number.
